O HESI A2 não é uma prova única e fixa. Ele é um conjunto de seções separadas, e cada uma delas tem nota própria. As seções acadêmicas são Math, Reading Comprehension, Vocabulary and General Knowledge, Grammar, Biology, Chemistry, Anatomy and Physiology e Physics. Há também duas partes não acadêmicas, um questionário de estilo de aprendizagem e um perfil de personalidade, usadas para orientação e não para a nota de admissão.
